You may receive a payment if you properly and timely complete this Claim Form, the Settlement is approved, and you are found to be eligible for a payment. The deadline to file a claim online is 11:59 p.m. PDT on October 16, 2022. If you send in a Claim Form by U.S. Mail, it must be postmarked on or before October 16, 2022.

You may receive a payment of either $75 (Tier 1) or $25 (Tier 2), depending on whether your Social Security number was exposed in the data breach. These amounts may be increased or decreased depending on the number of claims submitted.
